Restoration Psychology is an out-of-network provider for all insurance companies, which means we are a fee-for-service provider. We do not participate on any insurance panels. We choose to operate as a private pay practice to protect your confidentiality and to ensure we have the clinical freedom to treat you based on your mental health needs, not simply based on diagnostic criteria. 

However, Restoration can provide a superbill for clients to submit directly to their insurance provider for out of network reimbursement. Depending on your insurance provider, you may be reimbursed in part or in full and the timing of the reimbursement is dependent on your provider. With recent changes in healthcare, we've found that many policies now provide reimbursement for out-of-network mental health care, and we have many clients who are able to have their services reimbursed. 

Restoration Psychology is not a Medicaid provider. If you have Medicaid, we can provide you with a referral as we legally cannot see clients who have benefits under Medicaid. 

The preferred method of payment is personal check or cash, but credit cards (Visa, Mastercard, Discover, and American Express) are also accepted.

Payment is due at the beginning of your session. When you submit the new client forms through our online portal, we will have a card on file that we will automatically charge once the session has begun and you are present.

In accordance with Federal Guidelines and the Colorado Division of Insurance, all patients are entitled to a Good Faith Estimate which provides a detailed summary of charges. This Good Faith Estimate is good for 12 months from the date it was issued. If your care extends beyond one year, you will be given a new estimate. All sessions require separate scheduling and are done with your provider. Scheduling with your provider indicates that you are consenting to another session at the session cost listed above. The Good Faith Estimate is not a contract and does not require the client to obtain services from any of the providers or facility identified in the Good Faith Estimate. In addition, the Good Faith Estimate does not require the provider or facility to provide the services listed. Please ask for our handout for more information. 

Reduced rates are offered on a limited basis and is determined between client and therapist based on client’s household income and financial situation.  Restoration Psychology also offers clients options to see an intern/trainee for significantly reduced fees.

By providing a telephone number and submitting this form, you consent to be contacted by SMS text message. Message & data rates may apply. You as the client will receive these messages if you decide opt-in. The kinds of messages sent pertain to scheduling and cancellations.

Information We Collect
It is always up to you whether to disclose personally identifiable information to us, although if you elect not to do so, we reserve the right not to register you as a user or provide you with any services. “Personally identifiable information” means information that can be used to identify you as an individual, for example: When you visit our website or use our services, we may collect the following types of information:

  • Your name, date of birth, email address, phone number We will not use or disclose this information unless:
  • As required by law, for example, in response to a subpoena or search warrant
  • To outside auditors through who a Release of Information (ROI) from you have agreed to keep the information confidential
  • As necessary to enforce our Consent for Services
  • As necessary to protect the rights, safety, or property of Restoration Psychology, its clients, or others; this may include (for example) exchanging information with other organizations for fraud protection and/or risk reduction.

Other Information We Collect

We may collect other information that cannot be readily used to identify you, such as the domain name and IP address of your computer. We may use this information for technical administration of our website and/or research and development.

How We Use Your Information

We may use the information we collect for the following purposes:

  • Service Providers: We may share your information with third-party service providers that are HIPAA compliant who assist us in operating our services.
  • Legal Compliance: We may disclose your information if required by law or in response to lawful requests by public authorities.

Zoom
For communication purposes, we utilize Zoom for our phone and text messages with our clients. “In provisioning and operating the Zoom HIPAA Services, Zoom complies with the provisions of the HIPAA Security Rule that are required and applicable to it in its capacity as a business associate. Zoom is responsible for enforcing the administrative, technical and physical safeguards to prevent any unauthorized access to or disclosure of protected health information (PHI) in the Zoom environment”
( Zoom-hipaa.pdf). We use Zoom services as they are reliable, place a large standard on our clients security and privacy, and comply with up to date security measures to ensure safety.

External Data Storage
We may store your data on servers provided by third party hosting vendors with whom we have contracted and are HIPPA compliant.
Your Privacy Responsibilities
To help protect your privacy, be sure to take customary precautions to guard against “malware” by installing and updating anti-virus software to your devices(s).

Data Security
We take reasonable measures to protect your information from unauthorized access, alteration, disclosure, or destruction. However, please be aware that no method of transmission over the internet or electronic storage is 100% secure.
